Purpose: The Institutional Review Board (IRB) is comprised of four faculty members and a community member with no affiliation to the College. The IRB is designed to protect the rights and safety, as well as the emotional and physical well-being, of human research participants. All research involving human participants must be evaluated by the IRB; this goes for research being conducted by faculty, as well as for research being conducted by students under faculty supervision.
It is the responsibility of the primary investigator on any project to submit a proposal for IRB approval prior to commencing the research project. This submission must be complete and accurate in order for approval to be granted. Submissions by Cedar Crest faculty and students are done online.

IRB Request for Approval Form: Main request form used by researchers from other colleges and universities seeking approval of projects. This form must be printed and completed by hand. The completed copy will then be scanned, and emailed to the current Chair of the Cedar Crest IRB. The email must also include an electronic copy approval for the research from the Institutional Review Board (or equivalent) at the researchers’ institution along with the final version of the forms, which had been submitted for approval by the institution’s IRB.
IRB Research Description Supplement (Template): This document is in a format that allows you to download, edit (tailor to your own needs), and then print for submission with your proposal.
IRB Consent Form (Template): This document is in a format that allows researchers from other colleges and universities to download, edit (tailor to their own needs), and then print for submission with their proposal. One copy of the consent form will be given to each participant to keep for their records
IRB Consent Record Slip (Template): This document is in a format that allows researchers from other colleges and universities to download, edit (tailor to their own needs), and then print for submission with your proposal. One copy of the consent record slip will be signed by each participant; these must be kept for a period of at least one full year

IRB Rules and Regulations: This document presents the complete set of rules and regulations that govern research being conducted at Cedar Crest College. This document was constructed after consultation with local, state and federal guidelines governing research with human participants.
