Health forms are required for transfer students and SAGE students who live on campus. Please complete these steps before classes begin:

1. Open FalconLink → Student Health Records (first-aid kit icon) 
   • Complete all items under Forms 
   • You may use a physical exam from the last 12 months if you have one or you can download ours below to be completed.

Then you will need to upload your physical, immunizations, and insurance card:

1. Open Medicat in FalconLink to upload all your documents
