Health Forms are required for all traditional students (resident and commuters) and SAGE students living on campus. 🩺
All information is submitted electronically on Medicat, which can be accessed through Falconlink. You must submit health forms through the Medicat Health Portal by August 1st (fall entry) or January 1st (spring entry). Click here for instructions on uploading documents to Medicat.
If you are planning to live on campus, the Meningococcal Disease vaccine or waiver is required by state law. Students are not permitted to move in without either one.
