Residential students must complete these forms before being able to move in. All forms are still required for commuter students.

1. Log in to FalconLink and click the Student Health Records icon, which looks like a first aid kit. You should be logged in automatically when you click the icon. If not, use your FalconLink login and password.
2. Complete all forms under the My Forms tab.
3. Print and complete your physical exam form. You may use any physical exam completed in the last 12 months, even if it is in a different format.
4. Upload your physical exam form, immunizations, and insurance card under the Document Upload tab.

If you are having issues accessing the Medicat Portal, submit a Helpdesk Ticket outlining your access issue.
